Bacterial Vaginosis


What is Bacterial vaginosis?
Bacterial vaginosis is an infection occurring in the female vagina, in the female reproductive tract. In a normal female, the discharge produced by the vagina is white in color and it tends to turn yellow when it comes in contact with the air. Also, the normal discharge does not give off bad odor. Bacterial vaginosis is caused by overgrowth of normally occurring bacteria of the vagina, due to which the vagina starts secreting excess discharge. In a normal vagina, more than 95% of population is of bacteria known as lactobacilli. In a normal range this bacterium maintains the acidic environment of the vagina.  However, in Bacterial vaginosis the normal occurring anaerobic bacteria increases in concentration by a thousand times. This results in the lactobacilli decreasing in number. The pH of the vagina tends to rise between 4.5 - 7.0, hence the protection provided by the lactobacilli is reduced.  Bacterial vaginosis is the most common cause of abnormal vaginal discharge in women of child-bearing age. 

Symptoms of Bacterial vaginosis
There are several symptoms that are observed by a female suffering from Bacterial vaginosis. This includes changes in the texture and color of the vaginal discharge. The vaginal discharge turns from white to grey and becomes thin. The abnormal discharge gives off a bad smell. The smell is mostly noticeable during menstruation or during intercourse. The discharge is homogenous and mostly adherent to the wall of the vagina. Moreover, the patient feels irritation and itching in the area where the discharge comes in contact with the skin. Females also complain about dyspareunia which refers to pain felt during intercourse/sex. There is presence of light vaginal bleeding. Urinary systems are also associated with bacterial vaginosis, which include dysuria. Dysuria is the burning sensation felt while passing urine. This burning sensation is felt when the urinary area is also infected. 

Causes of Bacterial vaginosis
The bacteria responsible for Bacterial vaginosis are Gardnerella vaginalis, Bacteroides spp, and Mycoplasma hominis. However, the exact cause of why these bacteria start proliferating and what triggers them is still unknown.  

Risk factors of Bacterial vaginosis
There are multiple risk factors that make the female more prone to getting Bacterial vaginosis. Females who have multiple sex partners or have a new sex partner have a higher chance of getting the infection. Females who are using an intrauterine device for contraception or birth control also have a higher chance of getting infected. Females from certain ethnicity are also at higher risk, which includes black women and women who are non-Hispanic. Some females who naturally have less lactobacilli bacteria in their vagina also are at higher risk. Females who over-clean their vagina by rinsing it too much and using cleansing agents or scented agents in the pelvic region or while taking a bath in tubs are also more prone to getting infected. Bacterial vaginosis is also highly prevalent in women who are undergoing elective termination of pregnancy. 

Complications of Bacterial vaginosis
If left untreated or allowed to worsen, this condition can become worse and develop certain complications. There are a few complications that are associated with pregnancy. If the patient is infected during the pregnancy, bacterial vaginosis can result in premature deliveries in pregnant women and can also cause low birth weight babies. Bacterial vaginosis can also complicate into sexually transmitted diseases (STD). STDs include gonorrhea and chlamydia.  Also, if a patient goes through any gynecological surgery, she will be at higher risk of getting infected and develop a post-operation complication. 

Lab Investigations and Diagnosis of Bacterial vaginosis
In order to diagnose Bacterial vaginosis, the doctor needs to ask a few questions relative to your medical history. Also, they might conduct a pelvic exam along with taking a sample of the vaginal secretions. The diagnosis is usually clinically made by Amsel criteria. This Amsel criteria states that the pH of the vagina should be more than 4.5.  Also, when alkali is added to the discharge it gives off a fishy smell. The alkali used is 10% potassium hydroxide. The discharge should be grey and thin. Also, the sample that is taken for observation under the microscope should include "clue cells". It can also be diagnosed by Gram staining. 

Treatment of Bacterial vaginosis

The tea tree oil can be applied by adding few drops of oil in the bath or it can also be used in a pessary form. A pessary is a tablet containing oil that be manually placed in the vagina. 

Precautions and Prevention of Bacterial vaginosis
In order to avoid getting bacterial vaginosis, the person should keep limited sexual partners. Vaginal hygiene is very important. Rinse the soap from the outer genital region and dry the area after taking a shower. Also, avoid harsh treatments such as douching the vagina.

Back Pain

What is Back Pain?
Back pain is also known by the name dorsalgia. Dorsalgia is the medical term for back pain. The back region of the person’s body is painful during this condition. The pain can arise from the muscles of the back. The nerve supply of the back could be affected in this condition. Bones and joints on any part of structure of the spine with a problem can cause back pain or dorsalgia.

Symptoms of Back Pain
There are several symptoms experienced by the person with back pain. A few people lose the ability to stand straight as their lower back muscles get a severe spasm, hence making it difficult for them to move or stand. Persistent stiffness or pain can be experienced in the spinal region. A few patients experience sharp pain occurring in the neck or upper and lower back. Pain in the middle or the lower back can also be present for a long time, which is known as chronic pain.

The pain can radiate as far down as the buttocks of the person. This radiating pain in buttocks is known as sciatica. Different acts or postures can cause pain, including bending awkwardly, pushing, pulling something, carrying something, lifting something, standing for long periods, sitting down for long periods, twisting, coughing, sneezing, muscle tension, over-stretching, straining the neck forward and long driving sessions without a break.

Causes of Back Pain
There are several factors responsible for back pain. It could occur due to trauma or injury to the lower back. Bone and joint disorder like arthritis can also cause back pain. If there is a cancer in the spine, it can result in back pain. If there is an infection present in the spine, it will also cause back pain. Irregularities in the skeleton also result in back pain. Sciatica is also a major cause of back pain. Back pain can happen due to strain development in the back. The strain could be in the muscles or ligaments of the back. Any structural problem like collapsed disc of spine, bulging disc or osteoporosis can also cause back pain.

Risk factors of Back Pain
There are many risk factors associated with back pain. Smoking can increase someone’s chances of developing back pain. Being overweight and obese puts strain on the spine, hence weight is a major risk factor. Some people are prone to developing back pain due to their genetics, which they have inherited. Old age patients are also at risk. The female gender is at higher risk as compared to males. People who have a poor or bad posture are also prone. People who do physically strenuous work regularly are at a higher risk compared to normal population. A few jobs have hazards associated with them which can cause back pain, such as those that involve hard labour. Patients with anxiety and depression are at also risk of developing back pain.

Complications of Back Pain
There are a few complications associated with having back pain. Patients with back pain can develop Cauda equina syndrome (CES). Patients with back pain experience pain while sleeping, hence they have sleepless nights. This affects their daily activity and routine.

Lab Investigations and diagnosis of Back Pain
Your doctor will take a detailed history from you and conduct a general physical examination. The doctor will check the functioning of your back and your posture by making you sit, walk, lift your legs and stand. The doctor will also check your reflexes by using a plastic hammer. This hammer is not at all painful. If your doctor thinks that the cause of your back pain may be due to the presence of any tumour, an infection on any fracture is causing it.

Your doctor might order a few tests to confirm the diagnosis. These tests include tests for blood and urine. For that, you need to give fresh blood and urine samples. The doctor will also order an X-ray of your back. Advance tests can also be performed, which include CT scans and Magnetic Resonance Imaging (MRI). Your doctor may also order a bone scan.

Avascular necrosis


What is Avascular necrosis?
Avascular necrosis is known by many different names. These include bone infarction, osteochondritis dissecans and osteonecrosis. Necrosis refers to cell death due to any injury, inflammation or trauma. Avascular mean that there is a lack of blood supply. In Avascular necrosis, due to the interruption of blood supply, bone death starts to occur. As the effected dead bone part stops functioning normally, it becomes weakened and eventually it can collapse. Hence, this is considered a serious condition. Avascular necrosis is a chronic disease and it requires to be managed for lifetime if it occurs. It tends to worsen with time. 

Symptoms of Avascular necrosis
In the initial phase of the disease, there are no symptoms present. Eventually, however, the patient develops pain complaints. This pain can be felt in different part of the body. These areas include the hip joints, the knee joints and the upper arm bone. This results in limitation of movement of the body. 

Causes of Avascular necrosis
There are a few causes responsible for developing avascular necrosis. Any trauma to the bone or the joint can result in avascular necrosis. Patients with narrowed blood vessels can develop the disease too. If the pressure inside the bone is raised, it can result in avascular necrosis. Any fracture of the bone or a joint dislocation can cause avascular necrosis. The disease can develop without having any definite reason.  

Risk factors of Avascular necrosis
There are a few risk factors associated with avascular necrosis. Patients who are taking high dose steroids for any disease, for example prednisone, are at high risk of developing avascular necrosis. People who have had previous transplants are also at risk of developing the disease.   

Patients with cancer or diseases like systemic lupus erythematosus and sickle cell disease are also at higher risk. Patients who have HIV infection are also at risk. Certain metabolic diseases like Gaucher disease can also act as risk factors. Caisson disease is also a risk factor. Drugs like bisphosphonates are known to cause osteonecrosis of the mandible. People who have jobs which involve them staying in high pressure places, like the military and commercial drivers, are known to be at higher risk of developing avascular necrosis. 

Complications of Avascular necrosis
There are a few complications associated with avascular necrosis. The joints and bone can go through collapse. Also, this disease result in the patient experiencing extreme joint pain which results in them become immobile. Due to severe pain and collapse of the bone, the patient can become immobile within two to five years of developing the disease. This effects the patient socially, psychologically and also effects the economic or financial state of the patient. 

Lab Investigations and diagnosis of Avascular necrosis
After taking a detailed medical history, your doctor will do a complete general physical examination to rule out other similar diseases. In order to diagnose the disease, a few diagnostic tests are ordered. These include X-rays of bones and joints. More advanced technology is also used, which is known as magnetic resonance imaging. A bone scan can also be done, which is known as Dexa scan. In the early phase of the disease, the preferred diagnostic tests are Magnetic Resonance Imaging and bone scintigraphy. Autonomic Neuropathy


What is Autonomic neuropathy?
The body contains two nerve systems. One is called the autonomic system and the other is called the sympathetic system. Neuropathy refers to damage of the nerves. When the autonomic nerves are damaged, it is known as Autonomic neuropathy. The autonomic system is responsible for all the involuntary functions of the body. As the nerve system is damaged, the involuntary functions of the body are affected. These involuntary functions include the functioning of stomach and intestines. The urinary system is also affected. The functions of bladder become dysfunctional. The sexual functions also get deranged hence the penis is also affected. The heart also supplied by the autonomic system hence it is affected to. 

Symptoms of Autonomic neuropathy
There are several signs and symptoms experienced by a patient with Autonomic neuropathy. These include having a fainting spell or feeling dizzy while standing. This occurs due to disrupted blood flow as the autonomic system is responsible for this. You can also have problems with digestion. Patients can face sexual difficulties. The male patients can develop erectile dysfunction. In females, they can have lack vaginal secretion production during intercourse, making it painful.  Some patients go through sweating abnormalities. 

As the bladder is affected, one can experience urinary issues. Some patients experience intolerance while exercising. Lab Investigations and diagnosis of Autonomic neuropathy
Certain tests will be ordered by your doctor in order to diagnose you with autonomic neuropathy. Breathing tests are conducted. A tilt table test is done to check for any changes in blood pressure due to changes in position that are causing dizziness. Gastrointestinal test is done to check the functioning and anatomy of the body. A test known as Quantitative Sudomotor Axon Reflex Test (QSART) is conducted. To check the temperature dependent sweat production, the test known as Thermoregulatory sweat test is ordered.  A test for the urinary system of the body is done which includes urine analysis and urodynamic test is done to check the bladder function. Ultrasound is also ordered.

Autoimmune Hepatitis


What is Autoimmune Hepatitis?
Inflammation of the liver is known as hepatitis. Autoimmune is referred to a disease that is caused by our own protective mechanism attacking or fighting against parts of the body, thus having an internal immunity struggle. Autoimmune hepatitis is not a common disease. It is a chronic disorder. In autoimmune hepatitis, the cells of the liver get inflamed. This is known as hepatocellular inflammation. The cells then go through necrosis. Necrosis is when the cells start dying after getting injured. The injury could be due to an inflammation or other trauma. Necrosis can lead to cirrhosis, which occurs when normal tissue is replaced by fibrous tissue.

Symptoms of Autoimmune Hepatitis
Autoimmune hepatitis can show its symptoms at any phase of the disease. It can present itself in acute phase, chronic phase or when the liver is going through failure. There are several symptoms that can appear in Autoimmune Hepatitis. These symptoms include fatigue, or the person can develop fever, and pain can be experienced in the abdomen, muscles and joints. Itching is also a complaint associated with hepatitis. Along with itching, the patient can develop skin rashes.  The patient’s skin and eyes can turn yellow. This is known as jaundice. 

The liver can go through hypertrophy, which means it is increasing in size. The blood vessels of the abdomen become visible on the skin. The appetite of the patient decreases, which means they do not feel the need of having food to suppress hunger. The color of the urine can change from pale yellow or yellow to dark color. 

Causes of Autoimmune Hepatitis
The exact cause of autoimmune hepatitis is not known. But it is speculated that it occurs due to risk factors interacting with each other. These interactions could be between infections, due to inherited genetics or medications being taken by the patient. 

Risk factors of Autoimmune Hepatitis
There are several risk factors associated with the occurrence of auto immune hepatitis. These risk factors include (female) gender. Females are more prone to developing autoimmune hepatitis as compared to men. A person can inherit genetics from their parents which can make them prone to developing hepatitis. Autoimmune hepatitis causes several complications. These complications include hemolytic anemia. This anemia develops because the blood cells start braking at higher rate than they are made. Patients can also develop pernicious anemia. Pernicious anemia occurs due to inability of the body to absorb the nutrients responsible for the formation of the blood cells. Hence, the patient acquires a deficiency in the blood’s oxygen-carrying capacity. One can also develop ulcerative colitis. In ulcerative colitis, ulcers form in the colon of the digestive system. Patients can also develop rheumatoid arthritis and autoimmune thyroiditis. 

If the auto immune hepatitis progresses, it can cause liver failure. Liver cancer also has a chance of developing. Patients can also develop esophageal varices, due to which the patients start losing blood into the esophagus. Due to an increase in pressure in the portal vein of the liver, the blood pressure of the entire body can get raised.

In order to diagnose autoimmune hepatitis your doctor will order a few blood tests and perform a liver biopsy. In a liver biopsy, a sample of liver tissue is taken and observed under a microscope by the pathologist.

Certain diets and habits are harmful and further damage the liver.  You should eat food that has low amount of saturated fat. You should consume less fried foods and animal-sourced foods. You should avoid foods that contain sugar. Avoid eating processed foods and refined foods as they contain chemicals that can further have a detrimental effect on an already damaged liver.
